How to add a button on the right o bootstrap panel - html

I am trying to add a delete button on the right of bootstrap panel. No matter what I do it is not getting inline with panel heading. Help please.

form not submitting on localhost or remote

After submitting the form nothing happens, page doesn't reloads. Out of ideas.

to submit the form you will need to use <input type="submit"> instead of the <button> tag
once you update the code I am sure it will work, if you are using PHP it should be like this

How can I fix this weird line that comes with Boostrap model when having two div with col-lg-6 ?

I have a boostrap model , I see a weird line in it when having two divs with col-lg-6. Can you please tell me why and help me to fix it ?

I forget to place "col" divs inside <div class="row">.
I got it fixed by nesting "col-lg-6" divs inside <div class="row"> </div>

inline form in bootstrap3

I'm trying to have 2 input fields next to each other in a (bootstrap-css) col-md, but can't seem to get it right.
Here is what I have so far

It also doesn't help when I put both input fields in the same input group, by removing this:

How to make a partially inline Bootstrap form?

I'm attempting to produce this form with Bootstrap 3:
I'm having trouble making First Name and Last Name inline AND making them together take up the same width as the other fields. Currently I have the following
